Local Validation and Calibration Assessment of the CerviCARE Artificial Intelligence Cervicography Tool Within Routine VIA-Based Cervical Cancer Screening in Uganda: A Mixed-Methods Study with Comparative Patient and Provider Experience Assessment

Primary objective: To compare the diagnostic performance of the CerviCARE AI tool with VIA performed by experienced screening midwives and healthcare workers within routine cervical cancer screening services at the Uganda Cancer Institute and its affiliated satellite site in Mbarara, and to characterise the tool’s agreement, calibration and operational performance within routine service delivery.
Secondary objectives
1. To assess the quality-assurance performance of CerviCARE using colposcopy-directed biopsy and histopathological confirmation among a targeted subset of approximately 180 participants with abnormal, suspicious, borderline or discordant screening findings within the 1,500-woman validation cohort.
2. To assess and compare patient and healthcare provider experiences of routine VIA-based cervical cancer screening delivered with and without CerviCARE support, including understanding of results, communication, acceptability, trust, dignity, workflow, provider confidence and perceived usefulness within routine service delivery.
3. To quantify inter-observer variability in VIA interpretation among study providers and to assess how CerviCARE outputs align with consensus VIA and histology in discordant cases.

Health-related quality of life and environmental distress among climate-displaced communities in Uganda

4.1. General Objective
The general objective of this study is to characterize, summarize, and explore the connection between CRD and the current health-related QoL and environmental distress circumstances of CRMs in Uganda, focusing on both the national level and within a single IDP camp.
4.2. Specific Objectives
We propose two specific objectives that run in parallel to one another.
Objective 1: Characterize key stakeholders’ understanding of the causes and health-related consequences of CRD in Uganda. This objective will investigate stakeholders’ understanding of CRM circumstances in Uganda, particularly the types of climate factors contributing to displacement, the challenges faced by this population, and potential strategies for improving their health, using key informant interviews.
Objective 2: Explore how climate-related displacement experiences, aspects of vulnerability, and resilience contribute to quality of life and environmental distress (e.g., solastalgia and loss of place) throughout migration journeys of climate-related migrants in the Muhokya IDP Camp in Kasese District. Guided by an emerging method that blends geospatial mapping and narrative analysis,20 we will qualitatively explore the displacement, QoL, and distress experiences of CRMs.
