Developing and Testing a Registry-Enabled Strategy (ACT+) to Reduce Loss to Follow-Up Between Screening and Confirmation of Rheumatic Heart Disease in Uganda

To generate implementation learnings transferable to other ACT program sites globally, To identify contextual determinants of ACT+ adoption using mixed methods (Technology Acceptability Model (TAM) and Consolidated Framework for Implementation Research (CFIR) 2.0 frameworks)., To evaluate the effect of ACT+ on time to confirmatory echo and on linkage to SAP initiation,Aim 2 (Effectiveness-Implementation Trial): To evaluate the effectiveness of the ACT+ strategy in increasing the rate of linkage to confirmatory echocardiography among individuals who screen positive for RHD (primary effectiveness outcome). To evaluate adoption, acceptability, and feasibility of the ACT+ strategy among healthcare providers and program staff (primary implementation outcomes).,Aim 1 (HCD Phase): To explore multilevel barriers and facilitators to confirmatory echocardiography after a positive RHD screen among community members and ADUNU providers in Northern Uganda.
