Multi-level intervention for intersectional stigma reduction to improve HIV outcomes

Adapt the multilevel PRISM ISD-reduction intervention to address ISD for TGW in Uganda.

We will use the ADAPT-ITT model to adapt the multilevel PRISM intervention; specifically, the health facility TFA and individual-level ISD-reduction interventions (multilevel Ssinga nze intervention). Formative research (6 focus group discussions) with healthcare facility (HCF) staff and TGW at four health facilities ranked lowest on the PEPFAR Uganda stigma scorecard will explore ISD drivers and manifestations. We will use ADAPT-ITT to modify the multilevel Ssinga nze intervention comprising the HP+/HIVE3 interventions in consultation with community partners, peers, and providers. We will evaluate the feasibility and acceptability of the adapted Ssinga nze intervention using pre-post surveys, observation of trainings, and pre-post changes in mean scores on stigma scales.

Aim 2: Conduct a pilot hybrid type 1 effectiveness-implementation trial to test the preliminary effectiveness of the adapted multilevel Ssinga nze ISD-reduction intervention on (a) PrEP adherence or (b) viral suppression among TGW using a status-neutral approach.

Two of the health facilities in Aim 1 will be randomly assigned to Ssinga nze (the adapted HP+/HIVE3 intervention) with a 6-month wait-list control. TGW (N=120) will be randomly assigned to receive either Ssinga nze or wait-list control. All will enroll at study clinics, receive HIV testing, and be offered ART or PrEP depending on HIV status. TGW assigned to Ssinga nze will be followed at HIV clinics randomized to HP+; those assigned to wait-list control will attend control health facilities.

Aim 3: Evaluate implementation of Ssinga nze using qualitative methods.
Guided by the Intersectionality-Enhanced Consolidated Framework for Implementation Research [1], we will conduct in-depth interviews with purposively selected cadres of clinic staff trained in HP+ delivery (n=15) and TGW receiving HIVE3 (n=15), and 12 field observations (4 per clinic) to clarify how ISD-reduction intervention delivery may influence HIV prevention and treatment and to understand the actions and synergies of intervention components, assess health facility stigma reduction at HIV clinics for TGW, and interpret implementation data from Aim 2.

“’Peace through Health’ on the ground – an Ethnographic Peace Research (EPR) study of rural health workers serving refugee and host populations in Northern Uganda”

The research focuses on the roles and contributions of health workers in local processes of peacebuilding. Although frontline health workers are ideally placed to contribute towards peace and conflict resolution as first responders and key resource persons in their communities, their role in this regard has not been sufficiently researched or defined. The research aims at filling such gaps in our understanding, by exploring the experiences of health workers in- and near Arua, northern Uganda. One of the main goal of my research project is to contribute suggestions for how existing peacebuilding efforts of health workers can be supported and how their general peacebuilding potential can be strengthened.
