This is a multisite, multi-cohort study that aims to recruit subjects in four weight-based cohorts (i.e. Cohort 1, 2, 3 and 4), with each cohort having specific objectives aligned with it. The study will be implemented through a cluster randomize control trial, where a sample of treatment villages (clusters) will receive producer side treatments (free seed trial packs or discounted seed trial packs of a poorly adopted improved maize variety, Bazooka) while other treatment villages will receive a consumer side intervention involving cooking and tasting demonstration of maize from the improved variety. However, in both treatment and control groups, we will inform farmers about the existence of the improved seed variety and the benefits of using them, to be able to isolate the effect of the trail pack from merely knowledge effects.

The aim of this study is to provide IARC and the Ugandan Government with actionable recommendations concerning the development of paediatric biobanking guidelines. The primary objective is to identify the key ethical issues/concerns for researchers, laboratory staff, and health authorities in Uganda, specifically related to public trust in biobanking, and how trust/ trustworthiness could be addressed in paediatric biobanking governance as well as how the community might be engaged.
• Does it hold true that trust/ trustworthiness is contextual and that effectively would preclude any harmonisation of guidelines for biobanking? Or is there any common ground on which to build global /international guidelines,
• What are the gaps in biobanking in Uganda, particularly with respect to paediatric biobanking and,
• How can Uganda create paediatric biobanking guidelines that reflect the needs identified by communities and what would this look like, and,
• How can biobanks show they are trustworthy through their governance and what does that look like to Uganda? Is it sufficient to be transparent and accountable or are other cultural/economic/religious/aspects that parents consider in their assessment of trust in a biobank?

Promoting Universal Health Coverage for Amputees in Uganda through Social Enterprise and Engineering Innovation

The principles of patient autonomy, non-maleficence, beneficence, and justice will be employed. Institutional safeguards, such as human subjects review committees and policies governing organisational ethics, will be followed. To this end, we shall seek ethical approval from the Makerere University School of Biomedical Sciences Research Ethics Committee and the Uganda National Council for Science and Technology. We shall also seek permission from the Ministry of Health as well as the management of the health units from which the data will be collected. We will provide verbal and written explanations of the study, including the aim, the level of participation required, potential risks of involvement, and the approximate duration of participation. The consent will include full verbal (in case of illiteracy) and written/signed informed consent from all individuals and groups before accepting to be interviewed. We will seek written permission to record interviews, and access meeting minutes, reports, and other relevant documents from relevant persons prior to including them in the research. Participants will be asked to give verbal consent and confirm this by reading and signing the consent form (included in the appendix). The key informants, including service users, will be free to withdraw consent at any time without giving an explanation and without this negatively affecting the care they receive. Data collectors will translate information into the relevant local language where required. We will seek verbal consent from participants who struggle to read and write, and the next of kin will sign the consent form on behalf of the user. We will only include participants 18 years of age and older who can provide consent on their own. The participant information sheet will explicitly state that participation in this study is purely voluntary. Participants can withdraw at any time, up to 14 days after data collection, without giving the reason for their withdrawal from the study. Their decision to withdraw from the study will not affect the services they receive. Participants will have up to 48 hours after receiving full details about the study, to consider participating in the research and ask questions.

A Randomized Clinical Trial to Evaluate Solutions for the Management of Virologic Failure for Individuals on TLD in Sub-Saharan Africa.(RESOLVE)

Aim 2: Use simulation modeling to examine the clinical impact, costs, and cost-effectiveness of strategies to improve viral suppression after virologic failure on TLD. We will populate the previously validated Cost-Effectiveness of Preventing AIDS Complications-International (CEPAC-I) model with the novel clinical trial data from Aim 1 to project long-term clinical outcomes and cumulative lifetime costs. We will then compare the cost-effectiveness of the three strategies evaluated in Aim 1 for addressing virologic failure among people treated with first-line TLD in Uganda or South Africa. ,Aim 1: Conduct a randomized clinical trial to determine the optimal strategy for management of virologic failure on first-line TLD in SSA. We will recruit 648 adolescents and adults with two viral loads >1,000 copies/mL while on first-line TLD for at least 12 months, who are in care at one of six public-sector HIV clinics in Uganda or South Africa. We will randomize participants to one of the following strategies, stratified by clinic and prior NNRTI-exposure: a) Maintenance on TLD with switch to protease inhibitor (PI)-based second-line ART if virologic failure persists past six months; b) Individualized Care, with regimen choice based on results of genotypic resistance tests and urine tenofovir assays; or c) Immediate Switch to PI-based second-line ART. The primary outcome will be viral suppression (<50 copies/mL) at 48 weeks post-enrollment using the FDA snapshot definition. We hypothesize that rates of viral suppression at 48 weeks will be higher in the Individualized Care arm than in the Maintenance on TLD and Immediate Switch arms.,

Using community engagement to improve snakebite prevention measures in rural Uganda

The project aims to improve awareness and implementation of prevention measures against snakebite envenoming in rural communities in Uganda. In doing so our ultimate aim is to reduce the burden of snakebite envenoming in these high risk areas, using our local connections and community-integration approach to ensure the project continues to deliver this aim long after the end of the funding period.

We will achieve our aim via the following specific objectives:-
- Gain an understanding of current community knowledge and attitudes surrounding snakes and snakebite via interviews and surveys.
- Use the information from the first objective to prepare appropriate materials for co-creation workshops with the local community to promote awareness of local snakes and snakebite risk, implementation of prevention strategies, and discussion of appropriate responses in the event of a bite, all with attention given to targeted and specific solutions to local challenges and context.
- Donation of appropriate resources to assist local communities in reducing the risk of snakebite, such as appropriate footwear to prevent accidental bites on exposed feet and handling equipment to enable trained members of the community to move snakes away from high-risk locations such as school premises and homes (see final objective).
- Implement a 'train-the-trainer' programme to train the co-applicant in safe removal of snakes from premises and to enable him to take advantage of his teacher-training to pass these skills onto other willing members of the community. This will ultimately lead to a sustained network of members of the local community who can maintain risk reduction in the longer term.
