There is an urgent need for new and innovative research into the causes of non-atopic asthma (NAA), and the likely mechanisms involved. The objectives of this study are therefore to:
I. investigate the causes of NAA in a variety of settings: high and low asthma prevalence, HICs and LMICs (Work Package 1 (WP1)
II. investigate three possible mechanisms of NAA in these different settings, including
the relative importance of neural mechanisms (Work Package 2 (WP2))
This intended scientific value of study is to provide key ground-breaking insights into the
causes and mechanisms of the poorly-studied yet predominant non-atopic asthma phenotype
because: (i) non-atopic asthma likely has different causes from atopic asthma, and these have been little studied to date in both high-income countries and low- and middle-income countries; (ii) non-atopic asthma likely involves different mechanisms, which require different treatments.
This study is likely to show, in a variety of settings, that a substantial proportion of non-atopic asthma may involve different mechanisms, and will identify new directions and priorities for prevention, mechanistic research, and the development of new therapies

Promoting Universal Health Coverage for Amputees in Uganda through Social Enterprise and Engineering Innovation

The principles of patient autonomy, non-maleficence, beneficence, and justice will be employed. Institutional safeguards, such as human subjects review committees and policies governing organisational ethics, will be followed. To this end, we shall seek ethical approval from the Makerere University School of Biomedical Sciences Research Ethics Committee and the Uganda National Council for Science and Technology. We shall also seek permission from the Ministry of Health as well as the management of the health units from which the data will be collected. We will provide verbal and written explanations of the study, including the aim, the level of participation required, potential risks of involvement, and the approximate duration of participation. The consent will include full verbal (in case of illiteracy) and written/signed informed consent from all individuals and groups before accepting to be interviewed. We will seek written permission to record interviews, and access meeting minutes, reports, and other relevant documents from relevant persons prior to including them in the research. Participants will be asked to give verbal consent and confirm this by reading and signing the consent form (included in the appendix). The key informants, including service users, will be free to withdraw consent at any time without giving an explanation and without this negatively affecting the care they receive. Data collectors will translate information into the relevant local language where required. We will seek verbal consent from participants who struggle to read and write, and the next of kin will sign the consent form on behalf of the user. We will only include participants 18 years of age and older who can provide consent on their own. The participant information sheet will explicitly state that participation in this study is purely voluntary. Participants can withdraw at any time, up to 14 days after data collection, without giving the reason for their withdrawal from the study. Their decision to withdraw from the study will not affect the services they receive. Participants will have up to 48 hours after receiving full details about the study, to consider participating in the research and ask questions.

A Randomized Clinical Trial to Evaluate Solutions for the Management of Virologic Failure for Individuals on TLD in Sub-Saharan Africa.(RESOLVE)

Aim 2: Use simulation modeling to examine the clinical impact, costs, and cost-effectiveness of strategies to improve viral suppression after virologic failure on TLD. We will populate the previously validated Cost-Effectiveness of Preventing AIDS Complications-International (CEPAC-I) model with the novel clinical trial data from Aim 1 to project long-term clinical outcomes and cumulative lifetime costs. We will then compare the cost-effectiveness of the three strategies evaluated in Aim 1 for addressing virologic failure among people treated with first-line TLD in Uganda or South Africa. ,Aim 1: Conduct a randomized clinical trial to determine the optimal strategy for management of virologic failure on first-line TLD in SSA. We will recruit 648 adolescents and adults with two viral loads >1,000 copies/mL while on first-line TLD for at least 12 months, who are in care at one of six public-sector HIV clinics in Uganda or South Africa. We will randomize participants to one of the following strategies, stratified by clinic and prior NNRTI-exposure: a) Maintenance on TLD with switch to protease inhibitor (PI)-based second-line ART if virologic failure persists past six months; b) Individualized Care, with regimen choice based on results of genotypic resistance tests and urine tenofovir assays; or c) Immediate Switch to PI-based second-line ART. The primary outcome will be viral suppression (<50 copies/mL) at 48 weeks post-enrollment using the FDA snapshot definition. We hypothesize that rates of viral suppression at 48 weeks will be higher in the Individualized Care arm than in the Maintenance on TLD and Immediate Switch arms.,

Using community engagement to improve snakebite prevention measures in rural Uganda

The project aims to improve awareness and implementation of prevention measures against snakebite envenoming in rural communities in Uganda. In doing so our ultimate aim is to reduce the burden of snakebite envenoming in these high risk areas, using our local connections and community-integration approach to ensure the project continues to deliver this aim long after the end of the funding period.
We will achieve our aim via the following specific objectives:-
- Gain an understanding of current community knowledge and attitudes surrounding snakes and snakebite via interviews and surveys.
- Use the information from the first objective to prepare appropriate materials for co-creation workshops with the local community to promote awareness of local snakes and snakebite risk, implementation of prevention strategies, and discussion of appropriate responses in the event of a bite, all with attention given to targeted and specific solutions to local challenges and context.
- Donation of appropriate resources to assist local communities in reducing the risk of snakebite, such as appropriate footwear to prevent accidental bites on exposed feet and handling equipment to enable trained members of the community to move snakes away from high-risk locations such as school premises and homes (see final objective).
- Implement a 'train-the-trainer' programme to train the co-applicant in safe removal of snakes from premises and to enable him to take advantage of his teacher-training to pass these skills onto other willing members of the community. This will ultimately lead to a sustained network of members of the local community who can maintain risk reduction in the longer term.

Sample collection for the development of a new Abbott Rapid Diagnostics COVID- 19/ Flu A&B Rapid Panel

Influenza (Flu) and COVID-19 are both contagious respiratory illnesses - they share some similar symptoms but are caused by different viruses. Because some of the symptoms of flu and COVID-19 are similar, distinguishing infection between these viruses cannot be made based on symptoms alone. Diagnostic testing is needed to determine which of the two viruses is the cause of the illness.
Abbott Rapid Diagnostics is developing a new COVID-19 / Flu A&B rapid test device that can detect both flu and COVID-19 viruses in a patient sample and distinguish between them. The test is being developed on an improved lateral flow platform, to be conducted at the point-of-care. This study is intended to collect samples to aid in the research and development process for this new test. The study will provide standard-of-care PCR testing and potentially molecular sequencing for patient management at clinical sites in Uganda.

The overall goal is to study gastrointestinal symbiotic communities of African great apes and other non-human primates across Albertine Rift. We will explore relationships between the endosymbionts and the host organism as well as the relationships within the ecosystem. For the helminths, the impact on the host as well as epidemiology will be studied at the wildlife - domestic animals - human interface. Specifically:
Identification of parasite spectrum and infection burden in BINP great apes across the park and detection of possible reservoirs of zoonotic parasites at the BINP wildlife – domestic animal – human interface by screening selected animal hosts.
Evaluation of the role of strongylid nematodes in clinically ill great apes and investigation of the forces driving the zoonotic parasite infections by analyzing the environmental, host, and anthropogenic factors.
Characterizing the relationships within the endosymbiont communities, between the endosymbiont communities and the habitat, the relationships between the anthropogenic disturbance of ape habitats and endosymbiont communities and evaluation of the endosymbiont interspecies transmission.
Analysing the gut metabolite composition in non-human primates to detect molecular phenotypic difference between clinically ill animals and healthy individuals.
To increase the capacity of disease surveillance system in the area and to decrease the risk of parasite transmission between humans, domestic animals and wildlife, increasing the awareness of pathogen transmission in the local community.

1. To demonstrate the feasibility of monitoring cerebral tissue oxygenation (SO2), cerebral blow flow (CBF) and cerebral oxygen metabolism (CMRO2) among asphyxiated neonates and preterm infants in Jinja Regional Referral Hospital (Jinja RRH).
2. To determine evolution of post-natal cerebral hemodynamics and oxygen metabolism in asphyxiated and preterm infants in the first week of life. Specifically, we will
a. Perform several (daily) NIRS measurements over days on asphyxiated and preterm infants and up to two measurements on term newborns without evidence brain injury.
b. Compare these results in the first week of life with different etiologies of brain injury (hypoxia vs IVH) and controls.
c. Correlate NIRS results with the severity of brain insults determined by ultrasound.
H2.1 Asphyxiated neonates and preterm infants have lower Day 1 CBF and CMRO2 than controls.
H2.2 The decrease in CBF and CMRO2 is in proportion to the severity of brain injury.
3. To determine the predictive values of SO2, CBF and CMRO2 in neonatal mortality in the first week of life.
Gather data as in Aim 2.
H3.1 Cerebral hemodynamics and metabolism in the first week of life are sensitive biomarkers for mortality prediction.
H3.2 Changes in CBF and CMRO2 with age will add to the predictive value of mortality model at one week of life.
