Multi-level intervention for intersectional stigma reduction to improve HIV outcomes

Adapt the multilevel PRISM ISD-reduction intervention to address ISD for TGW in Uganda.

We will use the ADAPT-ITT model to adapt the multilevel PRISM intervention; specifically, the health facility TFA and individual-level ISD-reduction interventions (multilevel Ssinga nze intervention). Formative research (6 focus group discussions) with healthcare facility (HCF) staff and TGW at four health facilities ranked lowest on the PEPFAR Uganda stigma scorecard will explore ISD drivers and manifestations. We will use ADAPT-ITT to modify the multilevel Ssinga nze intervention comprising the HP+/HIVE3 interventions in consultation with community partners, peers, and providers. We will evaluate the feasibility and acceptability of the adapted Ssinga nze intervention using pre-post surveys, observation of trainings, and pre-post changes in mean scores on stigma scales.

Aim 2: Conduct a pilot hybrid type 1 effectiveness-implementation trial to test the preliminary effectiveness of the adapted multilevel Ssinga nze ISD-reduction intervention on (a) PrEP adherence or (b) viral suppression among TGW using a status-neutral approach.

Two of the health facilities in Aim 1 will be randomly assigned to Ssinga nze (the adapted HP+/HIVE3 intervention) with a 6-month wait-list control. TGW (N=120) will be randomly assigned to receive either Ssinga nze or wait-list control. All will enroll at study clinics, receive HIV testing, and be offered ART or PrEP depending on HIV status. TGW assigned to Ssinga nze will be followed at HIV clinics randomized to HP+; those assigned to wait-list control will attend control health facilities.

Aim 3: Evaluate implementation of Ssinga nze using qualitative methods.
Guided by the Intersectionality-Enhanced Consolidated Framework for Implementation Research [1], we will conduct in-depth interviews with purposively selected cadres of clinic staff trained in HP+ delivery (n=15) and TGW receiving HIVE3 (n=15), and 12 field observations (4 per clinic) to clarify how ISD-reduction intervention delivery may influence HIV prevention and treatment and to understand the actions and synergies of intervention components, assess health facility stigma reduction at HIV clinics for TGW, and interpret implementation data from Aim 2.

Specific Aim 1 will be to determine the median and interquartile range for the quantity of studies that non-expert providers perform prior to achieving a diagnostic accuracy of 90% and to document the amount of retraining required to reach this threshold across trainees.

Specific Aim 2 will be to identify patient, provider, or training program factors associated with trainees not reaching (<90%) diagnostic accuracy or taking a prolonged period of time to reach competency (as defined as being outside of 2 standard deviations of the mean).

Mechanisms of Immune Resistance to Mycobacterium tuberculosis

Aims: In this study, we will immuno-phenotype airway microbiome signatures as innate, Th1, Th2, Th17, or Treg-inducing using a chronic airway inflammation population. We will then determine the association between airway microbiome, M.tb-specific IFN-gamma responses, and M.tb burden based on GeneXpert status to underpin immune protective microbial signatures. Finally, we will validate identified immune-protective microbial signatures as a proxy for IFN-gamma reactivity and TB immune protection in a separate cohort of persistently QFT-negative individuals compared to household LTBI controls.

The economic impact of tax compliance interventions

This study focuses on three problems.

The first problem is documenting what costs companies incur when receiving different compliance actions. While anecdotal evidence suggests these could be substantial, there has, to the best of our knowledge, not been no systematic survey documenting the costs incurred by companies.

The second problem is estimating where to optimally target what compliance actions bearing in mind the costs to companies and spillover effects. This involves three steps, 1) estimate the impact of compliance actions on the targeted company, 2) estimate the spillover effects of these compliance actions on the targeted company’s’ trading partners, 3) conduct heterogeneity analyses to understand which groups of taxpayer experience the highest/lowest impact. Gaining a good understanding of this problem will help increase the benefit-cost ratio of enforcement interventions.

Finally, if we find that firms change their tax filings after receiving a compliance intervention, we will seek to understand what drives this change in behavior. Theoretically, the mechanism behind a firms’ response is unclear. Firms could respond because they update their beliefs about the likelihood of being caught for tax evasion, because they realize that being caught is costlier, or because the intervention imposes a financial burden on the company mechanically change the amount of taxes they pay.

MAGNET is seeking to test seven new agency measurement tools in Uganda as part of the MAGNET portfolio:
1. Administration of vignettes (stories depicting the extent to which husbands and wives engage in different modes of joint decision-making over large household purchases).
2. Administration of a few open-ended questions to capture women's decision-making should be recorded with high-quality audio.
3. Survey module to capture persuasion behaviours in the household.
4. Group-level module to capture women’s collective agency
5. Automatic cognition test on an electronic tablet to capture respondents' internalized beliefs about the suitability of women vs men engaging in certain occupations.
6. The critical consciousness scale captures respondents' critical reflection, critical action and motivation regarding household decision-making, work outside the household, and political participation.
7. Survey module to capture benefits from asset ownership.
These tools have been newly developed by MAGNET. In addition, we also plan to ask enumerators a series of questions on their background and attitudes before enumerator training to understand better how enumerator characteristics relate to measured outcomes.
