Closing TB Gaps using patient-centered approaches to improve TB Screening, Diagnosis and Prevention among people living with HIV

To assess the performance of novel TB screening algorithms among children,
adolescents, and adults living with HIV and presenting for routine care across a network
of family centered HIV clinics in Uganda and other four sub-Saharan African countries as
compared to the current WHO recommended symptom-based screening strategy.
To compare new diagnostic strategies leveraging the evaluation of multiple specimens
(stool and oral swab Xpert Ultra, early morning urine LF-LAM, point-of-care ultrasound
(POCUS), and a design locked Xpert Host Response Test against a reference standard
(Xpert Ultra on spot and early morning respiratory specimens) to improve diagnostic yield.
To compare the proportion of PLHIV who initiate and complete TPT with 3HP or 6H within
the context of a patient centered differentiated service delivery model allowing selection
of TPT regimen and randomly allocating enhanced adherence support versus the
standard of care for TPT and adherence support.
To evaluate the cost-effectiveness of screening and diagnostic strategies as well as
comparative TPT regimens for the prevention of TB disease among children, adolescents
and adults living with HIV

At this stage of my research, I would like to focus on three non-exclusive lines of enquiry: the debate surrounding the first identity cards during late colonialism; the documentation of non-indigenous people, and in particular the citizenship crisis of Asians in the 1960s and 1970s; and finally, the intense periods of identification during electoral periods, and more specifically the processes of voter registration.

Multisystem aging patterns in chimpanzees (Pan troglodytes schweinfurthii) living at Ngogo, Kibale National Park, Uganda

To better understand the biological and ecological forces that shape primate aging, we will assess both longitudinal and cross-sectional, age-related patterns of social, physiological, and physical variation in adult chimpanzees of the notably long-lived groups at Ngogo, Kibale National Park, Uganda. This project will integrate behavioral, genetic, endocrinological, immunological, and microbial data collected from wild chimpanzees. We will combine tried-and-tried observational and laboratory analyses with innovative, noninvasive omics-based approaches to studying physiological function. The proposed research will elucidate the advantages and limitations of wild chimpanzees as models of human health and aging and build an extensive multisystem dataset for future comparative chimpanzee research.
The three primary goals of this research are outlined below:
1) Characterize cross-sectional and longitudinal aging trajectories across functional domains. How do functional domains vary in the timing and rate of age-related decline? Do some systems (e.g., physical function, immune function) exhibit later onset and/or slower rates of decline than do others?
2) Determine the extent of sex biases in aging trajectories in Ngogo chimpanzees. Given that female chimpanzees at Ngogo have greater life expectancies at birth than do males [25], do female chimpanzees also show slower or delayed rates of aging across most / all functional domains? Are aging effects more pronounced in some functional domains than others, and if so, which domains show greater sex biases?
3) Develop methods to noninvasively study chimpanzee aging. Given that the study of wild chimpanzees is strictly noninvasive, we will continue to develop, validate, and implement new methods for studying aging across a range of functional domains. Notably, we will continue to refine the use of video recordings to measure walking speeds, which are an invaluable measure of physical function in humans and nonhuman primates alike [32, 33]. We will develop new urinary biomarkers for use in wild chimpanzee studies including biomarkers of neurodegeneration (e.g., Alzheimer’s-associated neuronal thread protein) and physical function (e.g., urinary pentosidine). Finally, we will validate and implement the study of genetic material from chimpanzee saliva samples, including bacterial and epigenetic information, by collecting and subsequently analyzing chimpanzee saliva samples onsite at Ngogo using transportable sequencing equipment (MinION Mk1C sequencer, Oxford Nanopore Technologies, Oxford, UK).

Using Implementation Science To Understand And Design Stakeholder Informed Innovative Interventions To Improve Youth HIV Prevention And Care Continuums In Rural And Urban Uganda

We will use platforms in aims 1 and 2 to strengthen capacity of implementation science and to translate findings into policy and guidelines through clearly defined and established channels. ,To assess the rollout of a culturally acceptable, feasible HIV prevention and care intervention among AYA at high risk, on uptake and persistence to CAB-LA or SEARCH-YOUTH treatment support intervention using implementation effectiveness hybrid designs. Aim 2 will be informed by Aim 1 with regards to specific implementation features. We will recruit AYA (15-24 years) both HIV negative and HIV positive. We will screen them for high risk of seroconversion if negative and for those testing HIV positive unsuppressed or at risk of being unsuppressed. We will assess scale up in real world settings of evidence-based interventions to increase uptake and persistence on CAB-LA (HIV negative) and increase viral suppression (HIV positive) among high risk AYA.,Using the updated CFIR, we will identify determinants (barriers and facilitators) of potential implementation success of two innovations: CAB-LA uptake and persistence and SEARCH-YOUTH (SY) implementation at the community, clinic, organization, provider, and patient levels in five CRPS in urban, semi-urban and rural Uganda. The aim of the study is to contribute a normative understanding and conceptualisation of child agency that moves beyond traditional assumptions to recognise the saliency of cultural and local normative conceptualisations
To achieve this aim, the primary objective is to formulate a new normative framework and conceptualisation of child agency in post-conflict reintegration based on the findings drawn from fieldwork conducted in northern Uganda.
The specific objectives to achieve the main objective are as follows:
1. To explore the conceptual and academic foundations of the key concepts inherent within the study, namely ‘the child,’ childhood, child soldier, agency, and reintegration.
2. To utilise the case study of northern Uganda’s former child soldiers and their experiences during both formal reintegration processes and informal participatory avenues to critically examine the applicability of the concepts to the reality of what is taking place.
3. To engage directly with FCS and their communities to draw upon their perspectives, realities, and actions to comprehend the nature of child agency and participation within a post-conflict society.
4. To determine the causal implications of a conceptualisation of child agency towards the formulation of reintegration programmes targeting children, particularly in relation to the incorporation of informal participatory avenues and local cultural norms within broader DDR processes.
5. To unpack the broader impact of the findings on the long-term sustainability of approaches to reintegration at a societal, regional, continental and international level.

Advancing methods for measuring agricultural productivity in household surveys

In a world with increasing climate change and intensifying food insecurity, agricultural productivity is central to development. Increasing agricultural productivity requires a detailed understanding of cultivated land and its limitations, and how these shortcomings can be addressed. Towards this end, getting more reliable data on land quality and quantity, two critical elements in agricultural production, is essential for accurately estimating agricultural productivity indicators like crop yield as well as for correctly identifying key agricultural relationships, such as the often-cited inverse land size-productivity relationship. Recent technological advancements both in the areas of in field soil measurement as well as GPS-based area measurement offer promises to this respect. However, there is a need to test and validate some of the most promising methods to assess their cost-effectiveness and potential for scalability in the context of large-scale surveys and vis-à-vis more traditional improved approaches such as the elicitation of subjective and observational information in household level questionnaires.
The primary objectives of this study are:
1. Improve the measurement of agricultural productivity in household surveys
2. Test and validate innovative methods for soil health and land area measurement in the context of household surveys
3. Assess cost-effectiveness of the different methods for scaling up in national surveys

Overall aim
To incorporate social behavioural research within a Sexual and Reproductive Health (SRH) and Human Immunodeficiency Virus (HIV) prevention programme to improve effective use of current HIV prevention options and inform the development of future HIV prevention and contraceptive technologies addressing unmet needs of women at high risk of HIV.
Specific objectives
1. To design support messages, based on a previous market segmentation analysis, for women at high risk of HIV infection and integrate them into a differentiated service delivery package in two urban sites.
2. To describe women’s relative preferences for use of HIV prevention and LA marketed contraceptive products.
3. To analyse socio demographic, behavioural and qualitative data and assess factors underlying preferences and reasons for changes in HIV prevention and LA contraceptive method choices over six months.
4. To analyse service utilisation and estimate the cost of delivery of a differentiated service delivery package (health service perspective)
5. To estimate the potential cost to service users of accessing the differentiated service delivery package (patient perspective).
6. To estimate the potential cost effectiveness of use of long-acting HIV prevention products in development if introduced within a differentiated service delivery package.

5. To determine whether the implementation of RUTF-box-based play materials for play stimulation improves nutrition program outcomes of children with SAM in Uganda. ,4. AMBULATORY BLOOD PRESSURE PATTERNS AMONG PEOPLE LIVING WITH HIV IN RAKAI, UGANDA.

Overall objective
The overall aim of this research project is to characterize ambulatory blood pressure, understand risks for non-dipping patterns, for early detection to improve cardiovascular outcomes among people living with HIV.
Specific Objectives
• To characterize ambulatory blood pressure patterns of participants 35+ years enrolled in the RCCS.
• To determine the prevalence of CVD risk factors by blood pressure dipping patterns among participants 35+ years enrolled in this cohort?
• To determine factors associated with non-dipping patterns among people living with HIV.
• To assess if participants with non-dipping blood pressure patterns exhibit greater burden of arterial stiffness.
• To evaluate the association between non-dipping blood pressure patterns and biomarkers of vascular endothelial cell activation.
